REGISTERING FOR PARATRANSIT SERVICE

Paratransit is a public transportation service offered to persons with disabilities who require significant mobility assistance. This page contains complete information on how to register for yourself or a person you care for.

To be eligible for paratransit service, users must meet the criteria set out by the Ministry of Transportation of Quebec in its Paratransit Eligibility Policy.

HOW TO APPLY

Applicants for paratransit service must provide the following:

• A completed application form : Print the document or complete it digitally by downloading a copy on your computer. The applicant must complete Part 1 of the form and Part 2 must be completed by a professional in the health care or school system with access to the applicant’s medical file.
• 2 passport-size photos
• Proof of age (such as a photocopy of birth certificate or health insurance card, etc.)

If a user wishes to authorize a designated third party to act in their name regarding STL services and communicate personal information, please fill out the following form :

Assistance on arrival

To ensure the safety of clients with an intellectual, psychiatric or other disability, a mention will be added to the user’s file regarding the necessary assistance to be provided on arrival. This is a mandatory procedure and signifies that a liable person has to take charge of the client on their arrival.

A client with a higher level of autonomy who doesn’t need assistance can obtain an exemption. If you wish to be exempted from assistance on arrival, please fill out the following form :

Are you a person with an intellectual disability or autism spectrum disorder (ASD)?

If so, please include a completed functional assessment form with the application.

MOVING OUTSIDE OF LAVAL? TRANSFER YOUR FILE

If you are moving outside of Laval and wish to transfer your file to another city’s paratransit services, please fill out the following form and send it to the STL’s paratransit service (address written above) :

APPLICATION REVIEW AND RESPONSE TIME

All paratransit applications are reviewed by a committee consisting of 3 members:

• 1 representative of the STL (the delegated representative)
• 1 representative of the person with the disability
• 1 representative from the health and social services network

If your application is deemed complete by the delegated representative, you will receive a response within 45 days. All committee decisions are unanimous.
 

REQUESTING A REVIEW

If you do not agree with a decision made by the committee, it may be possible for you to appeal to the review board. As set out in the Eligibility Policy, you may then notify the eligibility committee, which can update your eligibility status or the level of assistance to which you are entitled.
